MS ApplicationAll application processes can now be
completed on-line. You must complete the Graduate School
Application (linked below) and submit the application fee of $50 (make
payable to SIU and mail it to the department in care of the Graduate
Director).
In addition to
the Graduate School Application, you must complete the following:

Health Education
Program Application
- Minimum of 3 references (using
required form below)
- Official Transcripts from all
universities/colleges listed on your application form
- Curriculum Vitae
International Students Must Also
Submit:
- Copy of their passport photo

TOEFL Scores
The English language requirement may be met by
submitting:

550 on the paper-based TOEFL
-
213 on the computer-based TOEFL
-
80 on the internet-based TOEFL
-
6.0 on the IELTS

Financial Disclosure
Note that application due dates are as
follows:
- For entrance in the summer/fall
semesters, the deadline is the THIRD Friday of January. We
only admit students for Summer/Fall semesters.
In addition, it should be noted that
applicants who miss these due dates may be reviewed under certain
conditions. Contact the Graduate Director for more information.
